How we look after your information

Willowdene Rehabilitation Ltd

An easy-read guide

This guide tells you how we keep and use your information. It is written in a simple way. If you have any questions, just ask us. We are happy to help.

Who we are

We are Willowdene. We run programmes and support to help you.
When you work with us, we keep some information about you.

What information we keep

We may keep things like:

  • Your name, address, and phone number.
  • Who asked us to work with you.
  • Notes about your support and how you are getting on.
  • Information about your health, wellbeing, and things you have been through.
  • If a court has sent you to us, information about that.
We only keep information that we need to support you well and keep you safe.

Why we keep it

We keep your information so we can:

  • Give you the right support.
  • Keep you and other people safe.
  • Work well with other people who help you.
  • Show funders that our work is helping (we do not use your name for this).

Who we share it with

We only share your information when we need to. We may share it with:

  • Other services and people who help you.
  • A court or probation, if they sent you to us.
  • Doctors or health workers, if you need them.

We will tell you if we need to share your information. Sometimes, if we are worried that you or someone else is not safe, we may have to share it without asking you first. We will still try to talk to you about it.

We do not sell your information. We do not send you adverts without asking you first.

Keeping your information safe

We keep your information safe and private. Only the staff who need to see it can see it. We do not keep it for longer than we need to.

Your choices and your rights

You have rights over your information. You can:

  • Ask to see the information we keep about you.
  • Ask us to fix it if it is wrong.
  • Ask us to delete it (we can do this in some cases).
  • Change your mind if you said yes to something before.

If you want to do any of these, just tell us. We will help you.

If you are not happy

If you are not happy about how we use your information, please tell us. We will try to put it right.
